About Deviated nose (Crooked nose)
Deviated nose (Nasal septal deviation) can be diagnosed when nasal dorsum (nose bridge) and/or nasal tip is curved and/or not positioned on the midline of the nose. Most cases of the crooked nose are acquired rather and inherited, developed after the fracture of the nasal bone from such causes as impact from accidents. Crooked nose can be caused by deviation of one or more structure of the nose; nasal bone, upper lateral cartilage, alar cartilage, and septal cartilage. When there is deviation on septal cartilage, it often causes breathing problems such as nasal congestion (blocked nose), headaches, or sinusitis. Since severe deviation of nose leads not only to cosmetic dissatisfaction but also functional problems, Systematic diagnosis and correction of nose deviation is very essential. HERSHE plastic
surgery provides personalized surgery plan considering unique features of
patient’s nose based on nasal bone structure and degree of crooked septal
cartilage. |

Understanding Deviated nose Correction
Nose is a complex structure consisted of nasal bone, upper lateral cartilage, alar cartilage, and septal cartilage. When even one of these elements of nose structure is deviated, it creates deviation of overall nose. However, most of the cases, combined and complex causes crooked nose. Thus, deviated nose should be systemically corrected from both internally and externally, and diagnosis on extent, degree and direction of curve (deviation) is essential. Moreover, functional aspects should be reflected according to the external factors that causes deviated nose prior to the surgery. Most importantly, the condition of septal cartilage, which holds up nasal dorsum, should be clearly diagnosed for deviated nose. Septoplasty can be combined in case necessary. Since deviated nose has list of causes, crooked nose correction should be performed with comprehensive and systemic surgery plan by specialized surgeon who has profound experiences regarding the condition. Minor deviation can easily be corrected with simple nasal bone fracture and implant. However, in case of severe deviation, comprehensive and complex surgery methods such as septoplasty, lateral osteotomy, medial osteotomy and tip plasty can be performed.
Moreover, deviated
nose can be treated with different surgery method and the surgery result can be
vary according to the degree and extent of crooked nose and symmetry of facial
contouring. It is difficult to correct Deviated nose caused by facial asymmetry
in a perfect straight position, since midline of the nose is already deviated
into certain direction in accordance with facial asymmetry.In this case,
deviated nose correction will be performed to create straight nose as much as possible. |
Surgery methods of deviated nose
|
01. Correction with Nasal bone fracture
When the nasal bone is not straight, curved nasal bone can be corrected with the nasal bone fracture to strengthen the nose. |
||
02. Upper lateral cartilage correction
When cartilage from the mid-point of nasal dorsum to the nose tip is not straight, cartilage correction such as separating and reuniting upper lateral cartilage and septal cartilage should be carried to correct the crooked cartilage. |
|||
|
03. Alar cartilage correction When the nasal tip is crooked, lateral part of septalcartilage and alar cartilage need to corrected. |
||
04. Septoplasty Septum is vertical structure
that divides nose into two and located in the middle of inner nose. Both sides
of septum are covered with membrane and cartilage is located in the middle, and
septum holds up dorsum of nose. Symptom due to crooked septal cartilage is referred
as nasal septal deviation and involves various symptoms. Removal of the
deviated portion of septal cartilage and cartilage rearrangement will strengthen
the nose. |
Surgical Procedures
| |||||||
01. | Make an incision in the crooked nasal septum cartilage. |
| Cut the crooked cartilage to spread out and correct the shape. |
| Detach the outer layer of both sides of the costal bone with an osteotomy through the inner incision. |
| Press the detached bones to the center of the nose so that the crooked nose can be corrected. |
Post operation care | ||
1. | Possibility of bleeding for 1-2 days. Wipe with a soft tissue.
2. | Blow nose gently. | |
3. | Do not detach or cut gauze and bandage without doctor's consultation.
4. | Apply an ice pack on the day of surgery and the day after to reduce the swelling.
5. | Avoid drinking and smoking for a month after the surgery.
6. | Avoid wearing glasses or sunglasses for a month after the surgery. |
