About SMAS layer

 1) If the degree of sagging skin is not severe: MACs lift

 

MACs lifting (Minimal Access Cranial Suspension Lifting) is a type of face lift that reduces the length of the incision from the classical face lift method and improves saggy cheeks and jaw line. MACs lift is developed from the concept that pulling the skin tissue vertically shows more effective than the horizontal orientation. Well-defined and constant lifting effect can be achieved by pulling SMAS layer which is known as the main cause of skin laxity in the face.

 

MACs lift incises and remove sagging tissue in the fore part of the ear and weave soft tissue including platysma around neck and drooped SMAS layer under skin in a round shape by using special medical thread. Then, weaved tissues are fixed on the temporal fascia in the fore part of the ear to lift the sagging soft tissue upward. This surgical method is also called, ‘Double lift’ since MACs lift pulls not only the skin but also SMAS layer below the skin so that it is scaled-down and effect can last for a long time.​

1. Design the range of SMAS layer to be desquamated.

 2. After sleeping anesthesia, skin incision is made along the side of the hair line to the fore part of the ear.

 3. After desquamation of SMAS layer under the subcutaneous fat, it will be sutured after fixing by pulling the sagging tissue.

02) The serious stage of sagging skin : SMAS Lifting (Fasciectomy)

 

Fasciectomy is not only pulling the sagging tissue including the fascia upwards but also suturing and removing the sagging fascia after equal desquamation in a wide range. Effect will be permanent and certain due to removal and suture after desquamation of SMAS layer.

Although the degree of sagging on the forehead is not severe, deepen folded wrinkles can be formed caused by a repetitive movement of facial expression as well as aging of skin tissue. In this case, pulling inside the scalp after making incision can improve sagging eye brow, forehead wrinkles.

Temple

Temples located in the outer corner of eyebrow itself can cause lax skin and it also affects sagging eyes. By pulling the temple area with the incision made, lateral part of eyes can be much improved that cannot be altered by upper blepharoplasty or brow lift.

 

Cheek, nasolabial fold (mid face)

Mid-face area is the most visible area that aging sign is
too obvious in overall face. After making incision along the cheeks, hairline and ear line, skin laxity can be improved by removing SMAS and sagging skin at the same time.

 

Jaw line

With the skin incision made along the front part and the back of the ear, jaw line area and sagging skin under the ear can be improved. After removing lax SMAS, it will be pulled and sutured, then skin tissue will be sutured by eliminating unwanted skin tissue to raise the jaw line.


 

Neck

By making incision along the hairline at the back of the ear and pulling the skin around jaw line, horizontal wrinkles of the neck can be improved. Then, remove the loose skin under the jaw line by making incision.To gain a prominent effect of neck lift, incision along the hair line at the back of the ear and skin under the jaw line should be combined.

 

Combined procedures with facelift

The most idealized surgery result with overall improvement of sagging facial area can be expected if combined with three surgeries, Venus laser assisted lipolysis that removes sagging fat tissues downwards under chin, fat graft for face that improves loss of volume and blepharoplasty with improvement of sagging skin around eye area.
