What is SMAS Face Lift?
In facial structure, there is a special layer called SMAS. In SMAS layer, facial expression muscle, connective structure, and fascia are placed. Aging affects not only skin layer but also SMAS layer concomitantly. So when facelift is implemented, both subcutaneous fat flap and SMAS are stripped and lifted.
However, since facial nerve is placed right below the SMAS, it is not easy to strip only SMAS area without damaging facial nerve technically. Stripping SMAS layer is performed within salivary glands of inferior ear which is relatively safe and other areas are subject to safe lifting, using thread. For eye wrinkle areas and wrinkles in middle of the forehead, expression muscles in the SMAS layer are exterminated to eliminate those wrinkles semi-permanently.

Features of SMAS Face Lift surgery
Along the lateral hairline, inferior ear and back of the ear cheek, incision is made. Then we strip the drooping skin area widely enough, lift aging soft tissue layer below the stripped skin layer first, and lift skin flap for suture. At this moment, lifting direction is very important. If lifting direction is misled, end of eyes and mouth looks soaring, which are typically unnatural face traits after wrinkle treatment. Furthermore, blade-shaped ear-cheek of ear looks drooping- can occur by misconducted wrinkle treatment. Surgical Procedure
Incision line: along the lateral hairline, inferior ear and back of the ear cheek, incision is made.
After incision, flap is stripped - it is sufficiently stripped enough to reduce the tension in lifting, maximize the effect and leave less surgical scar.
By removing eye-closing muscle in shape of circular sector, wrinkles around the eyes and zygoma can be prevented. Deficient part occurring after removing muscle can be supplemented by incised skin flap in lifting or allotherm, which makes temple look full and face look younger.
Stripping SMAS layer is performed within salivary glands of inferior ear which is relatively safe. Fat (main reason for nasolabial folds) around the zygoma is lifted about 45 degree backward by using thread simultaneously.
The maximum lifting point is placed below the sideburns and skin flap is pulled toward the direction of arrow and extra remaining skin flap is removed along the incision line.
